I need a MySQL database with web interface that track doctor credentials. Web-based dashboard that provides group and individual gap analysis of credentials. In addition, the ability to provide 90-day, 60-day, 30-day warning of upcoming gaps (individual and group). Five credentials are tracking issue date, expiration date, and verification date. Sixth credential tracks number of education credits, number credits required, and verification date. There needs to be a simple password-protected web-based form to update data. The dashboards need to be password-protected also. Database will only be available via LAN, not Internet.
