ad space template for website

I have a current video player embedded within a very simple website. Because the player is flash the website was made flash. I need to insert advertisements in the website space (not the video player portion). I need to be able to change these ads on my own so I need a simple template where I insert the graphic and link it to another website. I also want to ad a pre-made survey template so that I can create surveys of my customers. I found these as possible sources. From the beginning of the project you requested to use pre-made website.
