Setup, code and design website with nomograms

We are finishing the analysis where we have constructed nomograms to predict survival for patients with throat cancer. We would like these data also to be availiable online.
You will have to design a very nice website that I can also easily modify afterwards (e.g. in wordpress or similar). Further you need to code and setup the calculations based on our data. We will supply this data within 7-14 days.

Regarding design:
It should be a simple staring page where you can choose one of the two nomograms we have. And then a page for each nomogram.
