Phonegap with Framework7 - adding PHP and JS SMALL WORK !! read description

Hello

I need some quick work to help me finish my student project, its very small and mostly all done.

This is dummy / demo app using phonegap and framework7. Some PHP and JS is arleady done and working. Note this is just student demo so not all features have to work. Points are described below.

The app has only few screens with basic features such as:

* Login - working

* Register - working

* Listing Doctors - static data

* Book Doctor - select date and time - partly working

* My Bookings - list booked appointments - need to be done

* Reschedule appointment - need to b done just to change date and time

* Re-book prescription - need to be done (prescription again is static data)

Database is DONE

Front-end is fully DONE

Functionality - login, registration is fully DONE

Book Doctor - is partly done

I need you to finish the outstanding features and maybe add some popup notification messages just so it looks good. Fix if there are any issues.

NOTE: This is very small work so no crazy estimation please. The max I am willing to pay is 70$. Good coder would not spend more than 2-3 hours max. I need this work to be done in max 3 days.
