Build Image Slider for Custom PHP Application

We have a custom PHP web-based application that we have built, but it has a very rudimentary image slider on the dashboard. We need this image slider replaced by something much more modern.
We need a piece of code to run an image slider inside of a table. The images should extend to fit the entire size of the cell, as the dashboard is viewed on many different sized screens. The slider should allow us to adjust how many seconds it delays between images, it should also allow us to add and remove photos. We are happy to simply have the photos named [login to view URL], [login to view URL], etc. No need for a image management tool, just the ability for us to change a setting in the code to indicate how many images OR you could simply designate a folder that all images are in and work through all those images. The slider tool should have buttons on it to allow the user to go back or forward in the images. The buttons are not forward and back buttons, but a small button for each image in the list. A sample screenshot is included. Lastly, we'd like the ability to set the text for each image and have the text (caption) for each image displayed in a bar at the bottom of the image. Again, see the uploaded sample.
Two images have been uploaded. The first (smaller) image is a sample of how we want the final slider to look. The second (larger) image is a screenshot of the current dashboard, so you can see how the slider needs to fit into that dashboard.
We cannot give access to the application as it contains private financial data, nor can we give access to our server or FTP. You will need to host this on your own server, and we'll implement it on our server and test.
