User Survey - Connecting Results to MySQL database(repost)

My website will offer portfolio analysis for users, but first requires me to get some personal information about them.
I would like to create a survey tool that would ask a number of questions to the users, and upon hitting 'submit' would dump the results to a mysql database and take them to another webpage.
The questions in the suvery will consist of questions like "age", "sex", and go as deep as "Look at the following scale [which will be a picture] and please rate where you fall on this scale?" and the user will choose from several choices.
All questions will be supplied.
Some major requirements:
- Survey must take place within the website (cannot link to an external site)
- Must have a design that fits within the overall design of the site
- once the user hits submit and the data is verified (all questions must be answered) the user is taken to another webpage that calls upon the database (the output can be a blank page stating the user's age)

The survey must include places for pictures - I will supply some sample pictures and an outline of the survey (about 15 questions). It should be a nice design that is easy to follow
