Logo & Application Icon For Mac OS X

I am a small one-man Mac developer and need graphical assistance.
Required:
1. Logo for my shareware 'brand' - suitable for use on web-site and in applications. Would be nice to have a simple 2-tone version e.g. Apple logo, and a more colourful one, based on same theme.
2. Application Icon for shareware on Mac OS X. As well as the icon, it will be used to make a 'Document Type' icon template, e.g. in PSD, so I can enter the filetype.
EXAMPLE: Toast Titanium, iTunes, QuickTime icon & file type icons, etc.
Icons to be supplied in formats (minimum):
- Vector
- MacOS X ICNS format (256, 128, etc.) with proper alpha and masking
- PNG
- Also a PSD Template ideally for the 'Document Type' icon.
Work for hire. Image release - Images and copyright are to be owned by hirer. No royalty payments, hidden fees, restrictions, obligations or licensing conditions are to be attached to the work.
You:
Experienced at drawing beautiful icons on Mac OS X, with an existing portfolio of icons you have drawn which I can look at.
Based on the bids / portfolios, I will PM you with more info (theme, etc.) for quick mock-ups before proceeding.
