549594 Convert Text File To MySQL Database

I have a text file containing info.

I need for it to be converted into a MySQL database.

Here is the format of each entry (3 lines)

LastName, Firstname MiddleInitial...............[Code] BusinessName (LocationCode)..........................................................O:

SecondFullName Address, City ZipCode-ZipExtended.................................H: (xxx) PhoneNumber

C: (xxx) CellNumber EmailAddress

Each entry may or may not contain any of this information.

It is very important that the info be entered accurately, without me having to proof read everything.

It is a long list, probably less than 1,000 entries but I don't know for sure. It could be longer or shorter.
