I need a simple program desgined to be used in windows for my delivery business

It will have 2 drop down boxes, show output button and will show an output

1st drop down box - list of restaurants

2nd drop down box - list of about ~15 zip codes

button - just to output on screen the delivery charge

So basically I select abc restaurant and a zip code and it will output what the delivery charge will be.

A excel file will be provided with all the data, it can be given to you in the format of the delivery charge from each restaurant to individual postal codes or in the format of each individual postal codes and their charges from the restaurant.

Very small simple project, I just dont have the time to do it, I will attach the excel file tomm with data I dont have it right now,
