Create CSV file from database

Requirement: A script that generate a downloadable CSV file containing a list of contacts along with the person (resident) they are associated with and the type of relationship to that person (resident). This information is stored in PODS tables in a mySQL database. The script should be written in PHP and runnable from a Wordpress page.
The CSV file should contain the following columns:
From wp_pod_tbl_contact:
name
date_of_birth
mobile_phone_number
next_of_kin
From wp_pod_tbl_relationship_to_resident:
name
From wp_pod_tbl_resident:
name
inactive
From wp_pod_tbl_care_home:
name
User interface: The CSV file will be created by selecting a menu option from a Wordpress website and then clicking a download button on a form. No other form fields are required. The file should be generated and presented to the user for download to their browser.
Coding: The script should be written in PHP and should use the following PODS api methods for database access:
findRecords() - [login to view URL]
fetchRecords() - [login to view URL]
get_field() - [login to view URL]
The script should only permit authorised users to run it. The following security check should be included at the start of the script:

Tables:
wp_pod_tbl_contact (a table of friends and family of residents)
wp_pod_tbl_relationship_to_resident (the type of link between contacts and residents, eg son, daughter, brother, friend, etc)
wp_pod_tbl_resident (a table of people staying in a care home)
wp_pod_tbl_care_home (a table of care homes)
