Database Extraction/Data Entry/Windows Scripting

[EDIT: FTP password changed] I need to extract the contents of a database CDROM into a CSV file or similar, suitable for importing into MySQL. The database uses a proprietary front end that does not have the reporting capabilities I require. The data files on the CD are Binary encoded and not from any recognisable DBMS so I have not been able to extract the data directly. The only way I can see to do this is manually (dataentry) or using some kind of script to screen scrape the data or perhaps use a 'capture print output to ASCII' driver and extract it that way. I will accept bids ONLY from coders who have downloaded and inspected the database. Please CONFIRM in your bid that you have downloaded and examined the database; I want an absolute assurance that you can handle the project which is now behind schedule due to a previous time waster. It is a self extracting RAR archive of the project database (frontend requires windows). Again, if you haven't downloaded and examined the database, please don't bid. I need the following fields extracted from the database, which contains 150,620 records. Company Name, Physical Address Line1,Physical Address Line2,Physical Address Line3,Postal Address Line1,Postal Address Line2,Postal Address Line3,Phone, Fax, Mobile, Email, Web URL, # of employees, and all SIC codes (standard industry classification codes) listed for the company (6 digit numbers). Not all records contain all field types.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
