ANSI 835 ERA Remittance Manager

I am looking to have a web-based ANSI 835 ERA Remittance Program developed. Coders should have knowledge of the ANSI 835 Electronic Remittance Advice File and how to parse into a MySQL database.
There are several components to the project:
a) Parsing of the 835 file into the database
b) Ability of the user to either download the actual 835 file OR view a human readable look-alike Explanation of Benefits (like a Medicare EOB) for the ERA file.
c) Files will be uploaded by the administrator for users. Users can only view their own files (a login system has been created, but this program will have to tie into the login).
d) Ability of the user to search the 835 files by payer name, patient name, date file was uploaded, check # and various other criteria.
e) Ability of the user to view and/or print a look-alike EOB for one patient rather than the complete EOB.
f) Ability to produce PDFs of the look-alike EOBs.
g) Coder shall completely install said program on web server.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
