Custom Wordpress Site involving javascript and PHP

Hello Bidders,
I would like a custom wordpress site created about nutritional supplements. The website is on my hosting account and has wordpress installed ([url removed, login to view]).
The purpose of the site is to rank each supplement listed by user votes. Each supplement will have 3 articles. One has the basic facts of the supplement, the second has the supplement benefits, and the third one has the supplement side effects. Each supplement will also have a list of key benefits and side effects attached to it.
PAGE TYPES:
There are three types of pages to this site: the homepage, Categories/Search page, and Post page.
(All three page types are attached as pictures with detailed instructions)
Homepage - Will have 6 categories listed. You will have to create pictures for each category. I will provide the categories.
Category/Search Page - Will display a list of supplements under each category or searched term along with a list of benefits and side effects. Will also display the rating of that supplement.
Post page: will have one of the three articles written about the supplement, the rating of the supplement and user opinions of the supplement. (PHP/javascript required)
Navigation Menu: Will have drop down boxes. The choices for each drop down are listed in the pictures below. (Should be similar in style to [url removed, login to view])
Side Menu: Each page type will differ, but they are all just a listing of different supplements (more details written in the pictures).
DESIGN/LAYOUT:
You do not need to create a header background, unless you want to. Just provide a placeholder for it.
The website should have a web 2.0 feel to it. Something like [url removed, login to view] or [url removed, login to view]
The layout must be similar to what I have attached in the pictures.
The design of the site is up to you, I would like a blue/green and grey color scheme.
