develop a web page using php, mysql and HTML, HTML5,JQ....

it should be responsive to pc browser and mobile browser, pc browser show html page and mobile show html5 page.

and

on the page, there are couple of input controls, email address, mobile, device(checkbox) and coupon code.

and

need do data-entry validation

if all right(coupon code is in DB and not actived.) passed sucessful message.

and then save the data on page to database(contains ip, device, datatime), and set the coupon code activated.

coupon code need to be generared to DB, about 1 million records.
