Need a simple app to show certain summary reports from MySql db. Currently need around 6 reports only. App needs to have following features :

1. User login form - to be validated from User Table

2. Show list of Reports based on rights given to the user in User Table

3. When a particular report is selected, then depending on the company & branch of the user show the details. Each of the 6 Reports will come from corresponding table.

For each report we need to have date filter before showing the report
