ADHD Screening Survey for iOS, Android, Website - Corona SDK vs cocos2d-x

I am looking for someone to use a cross-platform tool (whatever works, Corona SDK, cocos2d, cocos2d-x or something else) to have a simple app that will screen for ADHD. The app should be able to be published to the Android app store and the Apple's App store.

Attached are the documents for the program in detail. Essentially, you need both a parent and a teacher to take a survey to make the diagnosis. It is a big pain to get the teacher and parent surveys together, and this app will make it easier. It will be free for the parent to take the test initially, but if they buy an in-app purchase (or buy online), they will get a code to give to the teacher who will fill out a different survey. The teacher/parent should be able to do the survey on a website, an android app, or an iOS app.
