Design Work For Health Sales Page

I am looking for design work for a website designed at selling ebooks on natural health. This is what I want:
1 Header design (including simple logo) (550 x 95) (See attached samples for example)
2 advertisment boxes. Dimensions are 300 x 250 and 615 x 365. (see attached files for examples)
Book covers (13 small simple designs. (see attached files for example)
Sales page (see sample URL attached for sample, simple copy of this design)
I will supply all text/content. You supply stock pictures and do the design work. Must be able to achieve high quality work & design and have proven ability in the field of health, e-books on weight-loss, natural health, dietary supplements, etc.
If you do good work for a good quote I have plenty more work for you, I prefer to work long-term with the right person.
The Advertisement boxes do not need to be completed - the 2 boxes (300 x 250 and 615 x 365 do NOT need to be completed now as part of this job. I still need the Header, the book covers and the Sales Page.
