I need a door hanger made.

I would like it to be a 3 x 6.5

On the top- I need it to say Sorry We Missed You
Section #2 -See attached Image- (Date/Time/Check Boxes with words and lines)
Section #3 I want it to say Thank You
Section #4 Cardinal Lawns Logo- Attached (please center the words {Do not worry about the Bird})
Finally I need our Phone number on the bottom (614) 808-4448

I also need a backside that has Lines for Notes

I have attached two concepts as ideas. I like the grass idea and want something similar to that.
