Contest Brief

This is a contest to redesign a form process used to create automated telephone surveys. It’s a multi-step form, that presents a summary of the entered data at the end.

The user defines a number of parameters, uploads audio files, and specifies the DTMF (digits on the phone keypad) that correspond to answers for each question.

Full details of the existing system are included in the PDF, with multiple screen shots explaining a few key elements.

What I am NOT looking for is a simple rehash of the existing form. I want you to impress me with something functional, intuitive, and easy to use. How the form is presented to the user is important!

While screenshots may be submitted for discussion, contest entries MUST include a link to a semi-functional website, self-hosted -or- free-hosting sites are acceptable. I’ve had some great work presented on github.io. Contest entries MUST demonstrate the ability to upload files and provide a summary of the collected data. File uploads only have to capture the file name for the demo.

The demo website ONLY requires the necessary logic to complete the form. It’s to be considered a mock-up and there is no need for the final submit button to do anything.

One feature the users of the existing form process have requested, is the ability to batch upload the audio files. Please consider including this as a feature in the redesign. DropzoneJS could be a possible solution.

I would also like to point out, I am NOT interested seeing portfolios. This is a contest! Don’t tell me you can do it, show me! I treat contests like interviews, where the winning freelancer is normally considered for additional projects.

    Hello,

    The current form looks pretty neat. Is there any particular thing which was difficult to use, apart from the audio file upload? Something you'd like us to pay specific attention to?

      Great question! Besides file upload, the DTMF (keypad) portion is probably a bit bulky.

      Star, zero, and pound (hash) normally fall "below the fold". Users who use those option must scroll. Users who don't use the bottom keys, still have to scroll to reach "Preview & Save" on the last question.

      There is scrolling during step one too, but it's been cleaner. After entering the name and description, users can see all the remaining elements by scrolling to the bottom of the page. Although, step one 'could' be broken up to keep everything "above the fold".

      In the end, it's the UI/UX design that I am looking for. Not the back-end. Anything that's done in React could be ported over to something else. Use whatever you like.

      In the end, I'm looking for a HTML CSS demo. But if you're more comfortable discussing the design concepts before hand, then you can share some PSD designs. But as it's explained in the PDF, I need to see a semi-functional site.
