I have changed the wording for the questions. See new file I have uploaded. I still want the faces in-between
the questions and the scales as before, but use the new wording.
I now want a google doc for online use as well as a photocopy sheet.

Hi, I can make further changes in text and picture attributes including color, to #16 (second entry) and #7 as per your requirement. I have oriented to produce an easily re-editable assessment doc file.
