Contest Brief
Our client need to have a online form template designed, including working CSS.
It needs to be a template containing different Features (see below) as it will be used for different purposes.
Design style lead words is: Elegant, Modern, Simple and with good overview. Make sure the template form and the Features (see below) follow a coherent style.
The form template must be designed with working CSS parts that can later be moved around for the clients different purposes (questionnaires) of the form. The client will use the form on their webpage and is working in the legal sector.
FORM template instructions:
- Have a start (introduction)
- Have a questionnaire part
- Have an end (review of completed form)
- Create a cvs file with completed questionnaire information.
The form should contain the following Features:
- Text boxes (headline plus room to answer in text)
- Number boxes (headline plus room to answer in numbers)
- Headlines (normal headline or Statement text)
- Dropdowns (drop down menu to choose options from)
- Address form (containing normal address information)
- Yes/No style questions (question that could be answered by yes/no or other answers, like how radio buttons work)
- Multiple choice style questions (can choose more than one answer, like how tick boxes work)
- Question groups (main question and sub questions)
- Notifications for error/success (when right or wrong style of information is typed, e.g. text instead of numbers)
- Notifications for Completed/not completed (when a field has been completed with information or not)
- Mandatory field symbol (symbol that highlight required fields)
- Description information (boxes, space or other notifications for description in the template, for either the questions themselves or the form section)
The form must be presented in such a way that it can be IMBEDDED into the clients own webpage.

YOUR ENTRIES
