I need an order form to be used on my WordPress website. Order form should be similar to the one shared in the attached file. You can preferably use WooCommerce or any other WordPress plugin that you feel good for this purpose. Form should be easilty customizable by the admin from WP Dashboard, Discount option should exist and a variety of payment options including PayPal should exist. Please refer the sample/reference form in attached file, I will give you the new company name and prices once we decide to work together.
