redesign my existing website

I have an existing website ([login to view URL]) which I would like completely re-designing. We have been operating for 11 years and the website is stale, too long and follows no order. I want the website consolidated and appropriate for our changing world. I have some specific requirements. I want the font for our business name to remain exactly the same. I would like the dog logo to remain exactly the same and can provide you with the logo in whichever format you require. We have a registration form and 4 page questionnaire that we would like incorporated in the site to be completed, signed and submitted online. I would like 7 precise headings for the areas of the business. I have all the text and photographs for the site. I would also like a 3 minute, introductory video incorporating on the home page. I have a sketch/drawing of how I would like the home page to look I would also like all the reviews from our Facebook page incorporated on the site in a dedicated section. I would also like a link to our facebook feed on the home page. I am revisiting this site after a catastrophic and quite frankly depressing and upsetting attempt by a freelancer. If you are interested in the job i can provide you with everything that you need. The previous freelancer wouldn't provide any updates or work in progress and after 10 days waiting and a day before the deadline arrived presented me with a shambolic and hopeless attempt which bore no resemblance to my request. I am now extremely wary and you may not wish to consider this brief but I'm good to work with and I'm always available for any clarification. Thank you
