Build a Wordpress plugin that can be invoked through a shortcode. The plugin should present 3 questionnaires within total about 45 questions (divided over 5 pages) and produce a radar graph from the answers cumulated into 6 measurements. The answers are stored in a database attached to an ID and a day of birth (DOB). The start page invites to start the questionnaire or the possibility to review an old entry buy typing in the DOB and the ID. That way, it remains anonymous. The first and end page (where the radar graph is presented should be in a customizable Wordpress page. The questions, the graph and the calculation are provided in the attached Excel sheet
