PHP/PDF - Create a php/html script to generate pdf file

1) Look at this web page - Single Digit Addition Worksheets [login to view URL] 1) Design a simple form in Html Javascript with following options. 2) Choose first addend 3) Choose second addend 4) Number of problems is fixed - 4 x 4 - 16 problems - 4 rows, 4 cols 5) Language - always english 6) Text box to take a name 2) Create backend script - in php language, that generates random numbers according to input from above form. 3) Generate a Pdf sheet that takes inputs from step 2, and creates the worksheet. FPDF, Imagemagick are things to use. 5) Please bid appropriately, so we don't waste time in chatting. 6) Show sample php script that you have already worked on. 7) Once first order is solved, this is a long term contract with more than 50 pages to work on initially. 8) Budget is not more than 20-25$ for this project.
