PHP Insert Into code needed.

I am looking for someone to help me with some code that I need in PHP. I am looking to add multiple records to a MySQL Database based on the value of a field. When a record is added using an existing form there is a field named Repeat. If that field is populated by a numeric value I want the code to insert the same amount of records as is in the repeat value -1. So when a record is added if the field Repeat has 3 then it will add the original record plus two more records, if it has 10 it will add the original record plus nine more records and so forth. If the Repeat field is not populated then there will only be the one original record added. One of the values is a Date Field. I want the records to be identical with the exception of the Date field which needs to increment by the Week or Month depending on another field that has that value. So if the Repeat is 3 and the Frequency is Weekly it will incrementally add 7 days to the date for each of the additional records. If the Frequency is Monthly it will add one month to the date Field for the additional records. It is important that this code be an Insert Into statement. I can provide all the field names and values that need to be inserted, what I can not do is insert multiple records and change the Date field incrementally.
