We need a webpage of data form submission with following. This form should be responsive and error less with validation. There should be separate admin panel from where we can see the registered user info and download the same in excel. Registration info : First Name Last Name Mobile Email Date Of Birth (proper calender with year , month and date selection) Address District (drop down list of district) Taluka (drop down lost of taluka) Any criminal records - No (default selected) yes Aadhar Card No Passport Photo - User can select camera to take and upload photo, upload a jpg image Aadhar Card front side- Aadhar Card Back side - Submit User need to upload the working module on our server Have to submit the entire code. Proposal Validation is required
