Build the Secured Log-In and User's Dashboard (made of Forms submitted through JSON and MySQL)

Hi there! We are building the foundation of a new digital product to help Clinics manage their Patients' Forms safely, without the use of pen and paper. It's the first of multiple products that we have in the pipeline, so we are looking for recurring work. We are using Cognito for the form creation and we need to create a dashboard where the Clinics can safely log-in and access their patients' form and data, check if everything is alright and download/print the form if needed. We have a UX and HTML/CSS Designer in-house, what we are looking for is someone with basic PHP, MySQL and JSON knowledge to work on the data call and database. We are open to also build our custom Forms, without relying on Cognito, but since we need Signature and Image Upload features it might be overkill at the moment. Thank you!
