Build a form with admin in php

hello,
this project is very easy for any junior developer who knows php/mysql/html/css.
here i explain all my requirements step by step.
1: create a simple responsive html/php page with a form like attached image.
2: create a database in mysql with 1 table with the fields in the form.
3: when user enter all data and press submit button, run a emailverify api to verify email.
3.1: if api returns false, do nothing further and just show thank you message on page
3.2: if api returns true (mean email is ok), then save all form data in database
3.2.1: run another api using curl and submit the same form data to my marketing portal. (see attached image of 2nd api code)
after #3: show thank you message on page
4: create a admin login page that will login with admin/admin credentials.
5: after admin logins: show all the data of database table in page using datatable with default search & paging & export to csv option of datatable.
6: create another search option date from -> date to in same page
7: add a new page for admin password change
8: logout and go to login page.
that's it.
i want someone who start work now and complete it within a day.
