Create a wordpress instant quote calculator plugin

HI there. I have a wordpress site and need to create a calculator to provide an instant price on the screen to our customers. Basically, there will be a 'from' field with a drop down list with a number of locations across Australia, followed by a 'to' field with a drop down list with a number of locations across Australia. I can create a spreadsheet with the price I want displayed to be in the backend. So if someone goes to the 'from' list and selects 'Sydney' then goes to the 'to' list and selects 'Melbourne', then presses the quote button and a price displays on the screen for them (the price comes from the excel spreadsheet). I want to be able to change and upload the spreadsheet at any time and I want the customer to be able to email the quote to themselves by entering their email address. If their location is not on the screen they can select 'other' from the drop down list and it will take them to a quote form. If they are happy with the price on their screen they can select 'book now' and it will take them to the booking form. I want it to look something like the quoting form on this site: www.motorbikemovers.com.au. I will require the person who develops this to do the design work so that it's seamless and links in to our website perfectly. I'd also like someone to rebuild the fields in the quote and booking forms currently on my site. thanks!
