Create a Service Status script

We're looking for a PHP Developer to write a service status script for our website, with monitoring based on IP & Port. No HTTP service can be required on the monitored servers. We're looking to spend about $150 for the project itself, as it could be completed within a short timeframe using, for example, Laravel. The required functionality is below: - User System (ADMIN BACKEND) + Add User (Username / Password) + Delete User + Login - Server List (ADMIN BACKEND) + Add Server (Name, IP, Port) + Delete Server - Known Issues (ADMIN BACKEND) + Add Known Issue (Title, Description, Status [Reported/Investigating/In Progress/Resolved/Scheduled], Time Started, Time Resolved) + Delete Known Issue + Edit Known Issue - Server Status Page (FRONT END) List all Servers (Name, Status [Online/Offline] List all Known Issues where STATUS == Reported || Investigating || In Progress || Scheduled Link to show all Known Issues where STATUS == Resolved It is important that the views are somewhat easily editable, as we'll be tieing them into our existing design.
