Create PHP Codeigniter web application for PDF Form

I have a PDF form which is used to collect data. I need a couple of things in PDF form.I have already made the PDF form myself.I will send you form you can work on them.
(i) PDF form look is not good So i want align the all form field and give the proper name of form fields that developer can easily understand the form field in future.
(ii) the form to be able to be submitted to a url which will process the form data and push into a database, we application will be written in PHP
(iii) the mysql database create in my web server.
(iv)The pdf form large with 25 pages and 1400 form fields.
I would like to see examples of similar work before choosing so I can be confident of your ability to do this task.
