Create a Forum on My Existing Website - Very Urgent Job

I need you to create a forum that looks similar to: [login to view URL]
This will be added to an existing website of mine, but you can make it look the same as the URL I have provided.
1.) I need to be able to create my own categories. In this case I would need to be able to create my own new job categories, the same as the link I have provided above (the "category" above would be "Corrections Officer", etc.).
2.) I need to be able to customize the two sets of links that are located on the right column of the page when I create each category that will only show for that category.
3.) Newly created accounts need to be approved by me before being able to post.
4.) The main page should should be setup exactly like: [login to view URL]
(with the same functions, etc.)
5.) Forum posts within each category need to create it's own folder... so in the case of the URL above it would create a folder named /corrections-officer/...name of forum post created.
(the option for me to create the name of the URL for each category I create would be ideal).
