Landing page design

Attached is a sample image from one of our current landing pages, I am looking for someone to create their own rethink of how that landing page should look... Landing page must accommodate video and have an opt-in box on the right side as well as a phone number and logo in the header.. Your bid must include all design work as well as coding to html.. content areas must be tagged and easy to identify in the code.. Delivery also to include PSD of the design and a separate PSD for the header so I can easily modify it as well as a copy of all fonts used. This is a test project to find a designer to create 4 more landing pages for us.
