Custom form/app on Joomla to accept student enrolments

We are after a custom joomla app (in Fabrik or similar) that will handle enrolments for courses for us (not LMS however, that is external to website). The app needs to have a form that collects specific information required to apply to the course and place in the database. It then needs to send an acknowledgement email and also place them in our mailchimp marketing list (with a 'interest' applied for marketing segmentation). Mailchimp list is already established. The form must be able to accept uploaded documents, which we need to be able to view when we login. We then need to be able to log in to review and accept application, which would then send an acceptance email. Some applications are auto accepted, so they would just need to auto change status to accepted. Once a student enrols (this is handled externally unfortunately and cannot be changed at this time) we need to be able to login and change their status to enrolled. They then need to be removed from mailchip (or moved to another list) to stop pre-enrolment marketing, and an enrolment confirmed email sent. We need to be able to edit forms ourselves in the future, as well as the emails it sends. We need to be able to look at stats from this data, applications vs enrolments, time to enrolment etc. Hence why we think Fabrik may be the ideal software to make our mini application.
