Public Voting Form.
I require a public voting form so the public can vote for a nominated name, this form will then be sent to my email address; the form must be easy to update so I can add or delete names (preferred from a txt/xml document) from at least 15 categories.
Each category will have around 10 names placed along with a text box for the public to add another name (if they wish to do so).
Only one name can be selected from each category along with a select box called ‘other’ where the public can enter another name (text field).
Once the public has voted a confirmation message will display showing them their voting selection and for them to confirm, once confirmed a message will display that their vote has been submitting.
An email will then be sent to my inbox with the results and to the voter who has submitted the vote (voters email must be able for me to customised, i.e. I wish to add additional information).

The voting form will be placed inside a movie clip that will then be placed on a time line (I’m using Flash8), I’m happy for the form to be using php and not MySQL.
What is not required is a results display.

The voting movie clip will be placed inside a pop-up flash window in the flash swf file so all we require really is the voting form (270px wide, white background, height doesn’t matter as we have a scroll bar in the pop-up).
The most important thing is that we can add/delete the names under each category (15 in all) as the votes come in (we can change things manually).
