Simple One Page Website

Seemed to have lost my original bid request so anyone who bid on it, please do so again Project is for a one page website that will have a questionnaire of about 6 questions for customers and have the ability to upload the responses as well as a photograph. After receiving both the questionnaire and the photo, analysis will be done over a 2-3 day period and the a report will then be sent to the customer. It will have a database and provide for both manual and automated delivery. The questionnaire must be able to be sent to one of the partners in the business and that person will then send the info to the other partner. The 2nd partner will then send the report back to the original partner when the analysis is done and the report and an ebook will be sent. I also need the website to be integrated with an existing blog. I obviously have the domain name, the hosting company and the header. Your suggestions on the technical aspects. ## Deliverables A one page website that meets the requirements above: 1) Ability to have a questionnaire filled out by a customer and the answers as well as a photograph uploaded. 2) A database to keep track of all the information sent and received. 3) Tie in with the existing blog. 4) Uploaded with header in place to host. ## Platform Your suggestion
