216580 ONLY ONE PAGE JOB (HOME PAGE)

We are looking for someone to design a new home page for mikesplace dot ca that is an actual login page with login and password along with video like what they sell at livefaceonweb dot com that has what looks like a live person that just says "Welcome to Mikes place please enter your username and password in the boxes above, if you don't have a username or password please contact Mike. Thanks for visiting". This is a single page authorization page with 1 flash or java not sure best way to do the video but not too complicated want to keep costs down as this customers site is just for family & friends to keep in touch.
