220744 Three Simple Forms

Require 3 simple forms for my site. They must be styled to match my existing site. Please see attached screenshot. I have full PSDs and can send any design/ styling etc you may want. THREE FORMS: 1) Simple Contact Form [contact name] [contact email] [contact message] Captcha Optional. 2) Web Site Feedback Form [contact name] [contact email] [how did you find us?] [comments about our site] 3) Newsletter Signup [name] [email address] Note: There is nothing complex about the newsletter signup. I simply want to receive a message with someone's email address in it. I will do the rest and make my own database later. That is it. I might also need help integrating it in the site, but I'm good with CSS so I shouldn't need much help. THIS PROJECT IS URGENT! I require these forms TODAY. Please assist!
