Simple rename application for mac

Hi,
I need a simple rename app for a mac where I can change the names of all files in a folder.
1. Select the folder
2. Define a prefix (optional)
3. Define a extension (optional) - overwrite
All files should be renamed with the date (changed) - the dateformat is always: yyyyMMdd_HHmmss (20140901_100520 - 1th Sep. 2014 10:05 and 20 seconds)
Example: prefix "H_" + extension "mp4"
File: [login to view URL] --> "[login to view URL]"
No prefex --> [login to view URL]
no extension. [login to view URL]
no extension or prefix: [login to view URL]
need an executable file for mac and when all is working the sourcecode.
