Google maps road segments -- proof of concept

I need a proof-of-concept web app.

* The app should list the five road segments below.

* A checkbox should appear next to each road segment

* The app should have a button labeled "Map Now". When clicked, all the checked segments should be shown on a Google map page.

* A simple info window should appear on the map for each segment (it is ok if you have to click on something to make the info window appear. The info window content for each segment is given below.

All segments are in the vicinity of Waitsburg, WA, USA.

Here is the list of road segments:

1. Bolles Rd from Preston Park to Medken Rd (info window content: "Hello World")

2. Medken Rd from Bolles Rd to Lower Whetstone Rd (info window content: "Welcome to Medken Rd")

3. McKay Alto Rd from Lower Whetstone Rd to Kellogg Hollow Rd (info window content: "Something useful goes here")

4. Kellogg Hollow Rd from McKay Alto Rd to Upper Whetstone Rd (info window content: "Fun road - 5 stars")

5. Upper Whetstone Rd from Kellogg Hollow Rd to Eager Rd (info window content: "This road is also called Whetstone")

I will need instructions for how to edit and add to this list of road segments. Editing a text file via ftp is fine.

The app should be installed on a Linux VM at eApps. I will provide the login details.
