Custom ipad 2 electronic forms for auditing

I currently have a form for the iPad for auditors to fill out while they are conducting in field audits on technical installs. The form needs to cater for the insert/capture of images/photos, attachment of files, electronic signatures, GPS (Google maps) reference to the address the audit was conducted, A basic degree of logic, i.e. if you select A then B,G,Y are filled out etc. The form need to export into a SQL database and have the ability to export as a PDF and email also need two levels of access Admin where you can create, modify forms and user who can only fill out the form and send
