Microsoft Word Form to Excel Solution

We would like to have a Microsoft Word document form use the form data as fields to be extracted to to Excel. This process must enable the initial template to be used with other Word Documents that will have the form data available and able to transfer the process/scripting to other templates as new ones are created in the future. Basically, the intial tempalte needs to work on other templates that are similar. The initial template will be one page but actual Word form will have many pages. The code and process when complete will be applied to existing documents requireing data gathering and forms as contracts. The execution of the Word form to excel extraction should be made with a small web-based GUI (run locally) that allows the selection of the Word form, and then does the extraction of the form field data, exports to an excel file and saves as a CSV/TXT/XLSx (option to select) with the field names in the Excel document populated with the data input from the Word doc. This data will be reviewed by the person running the process and teh end state is that the spreadsheet data from the Word Form will be imported to a database. The solution will also enable the process to be copied / installed to other machines in order for the process to work on mulitple machines. Solution will also need to run on Windows 7 and 10, with the minimum Windows Office being 2010. Source code must be supplied and documentation on the source code to allow for future changes and updates. We are also open to alternative ways to make this project work with limited user input and maximum automation using existing sfotware for the project.
