Create Sharepoint Template and Create Multi-Approval Infopath Form That Saves to Sharepoint.

This project is two parts: 1. I would like someone to create a SharePoint template that follows this theme:([login to view URL]). It needs to be fully functional on our SharePoint site. All we require is the template created, we will do the content part. It needs to have a stationary menu on the left and the ability to put content on the right along with edit the header and footer content. 2. The second part is I need a web form created that matches the theme in part 1. The web form needs to be an InfoPath form and included multiple levels of approvals based on roles setup in Sharepoint. Sharepoint sends approval request to the next person by email once the first person approves. Three levels of approvals are required, and when all approvals are in, the form saves a copy to SharePoint archives. The form needs to look good and incorporate all of the features seen in the attached sample. The overall look should match the template in part 1 but be designed to the sample document attached. When bidding, please include in the subject line that you have "read these requirements, and the budget is adequate." I will pay 33% as a milestone in advance, 33% when the design is visible on your server, and the remaining 33% when the project is done and functional on our server. Needs to be completed in 14 days.
