Conversion of Word doc to a Word form.

We created a Word document for a project that is ideal for a paper based questionnaire, the challenge arose when it was posted to the CRM for use and editing (using edraw object), the text boxes we had used created a problem for us, scrolling of the text resulted in it being of no use to us. We need the document to be rebuilt into a "form" type format using tables were, ideally the layout remains the same, but the data can be typed into the forms fields, and can be captured in the document when saving back to the server.
