CSS and xhtml 3 page form W3c standard

Please request furture details for this project if you need to know more to give the correct quote. I need a css and xhtml guru who is fast to complete this work and future work needed. I am a php developer and have attempted to create a 3 page customer form within exsisting website (pm me for domain) for a client. However the css is wrong and does not display in all browsers. I need someone to redesign the form with css and xhtml to make it look progressional. The main theme and colours are provided and the layout can be seen with the attached 3 pages. You do not need to touch php even though there maybe some in the example files attached. I also need this designer to be on tap via chat service as customer service to my questions are vital as this is an urgent project - your must have outstanding communication.
