Create 3 Online submittable forms

Forms have already been created in Word, need these forms to be created so customers can fill out the form online and submit the form to us via email.

These forms must fit in with the site branding and should be in pdf format but with buttons at bottom of the questionnaire to submit or to print.

2 of the forms are 2 pages of A4 in length whilst the other form is approx 7 pages in length.

I have attached an example of how I want the form to function
