ASPX web application in VB.NET (not C#)

Ethnium is looking for developing an ASPX web application in VB.NET (not C#) where we can:
1) Enter suvery information, as question and potential answers, as for example:
a. Who is your preferred candidate for the next election?
i. John Doe
ii. Jane Doe
iii. Joe Smith
2) That information should be stored into a SQL Server 2012 database
3) Different types of fields can be selected when entering a question and a potential answer, as for example, dropdown boxes and radio buttons
4) Free entry fields (textboxes) must be allowed, as for example:
a. How old are you?
b. What is your monthly income?
5) We need to set a workflow for the questions. For example, on point 1) above, if the user chooses “John Doe”, we could ask questions specifically for that answer. Example:
a. What is it that you like about John Doe?
i. He is honest
ii. He is brilliant
iii. He is a nice guy
iv. Other
6) The application should consist of two different sections:
a. Survey setup (no need to be mobile)
b. Data collection module (mobile only). This is the web application our people will use on the streets to collect the data. This will be accessible through a web link and will be used on the phone’s browser. This IS NOT a native mobile app.
7) We have a table of Customers. Each customer has a contract number. A contract is basically a survey. Both the surveys and the results should have two fields: CUSTOMER ID and CONTRACT ID. This is because the same customer may have more than one survey (contract) at a time.
8) We have a table of Data Entries, each data entry will login to the web page (6-b above) using a username, password an contract number
9) Results should be downloadable on a CSV file in a table format, for further analysis by our analysts.
10) Surveys WILL NOT be placed on any website, this is an application we need for our people to collect data.
