Azure-Services pattern for existing MVC 4 software

Hello, I am looking to a source code for a small basic software under Azure platform, satisfying the following objectives below, also either a copy of source code deployed into Azure account for demo purpose, or ready to be deployed and tested: Front End: ======== *) No any complementary UI cosmetics, just basic HTML controls *) Only 3 pages, page for login (username and password), page for showing data as plain text (id and description) and page for add new records (id and description) *) Links between pages as following: -) Login page to show page after success login -) Show page has a logout link, which ends the user's session -) Show page to add page -) Add page to show page after save or cancel *) Dummy logo image on the 3 pages *) Current login's date and time should appear on page for showing data and page for add new records, the value should be maintained accross all different requests on different instances, the value should be maintained through session stored in storage (explained below) *) User's session timeout is 1 minute, after that it should logout and redirect to login page automatically Back End: ======== *) Should be three web roles instances with load balancer to handle user's requests *) web roles instances must be stateless, as load balancer exists, the response might go to different instance where the request sent *) Session information should be maintained through "Table Storage" or "Cache storage", not in memory, as it should be shared between all instances *) Data (id and description) should be maintained through SQL Azure database
