We require a tool to capture some information and to retrieve the required information (using search criteria) as and when needed. Ideally, the front-end of the tool will be a VBA form with fields to capture the required information and the backend will be Microsoft Access Database(2007). The VBA form will need to have a “Submit” button, so that once the user fills in all the fields and clicks it, all the information will go to the Access Database. The form will also need to have a search field so that we could fetch the data using any of the search criteria (eg: First Name, Last Name, Address 1 etc.) and could be exported in the form of an excel spreadsheet. In addition, the tool will need a user credential system in place, so that only the admin(s) could fetch the reports and others could only do data-entry

did something like this recently to be uploaded to our sharepoint. this should be a quick fix. And if i can understand how this will be used we can look at report creation, etc

I have developed a sample vba form, which can feed information in to Access database and can retrieve database information and display on excel. I have done so many similar task.
